Behavior Support and Management
Each individual receiving 245D services has a Positive Support Transition Plan (PSTP) or an Individual Behavior Support Plan (BSP) developed in collaboration with the person, their guardian (if applicable), licensed professionals, and interdisciplinary team members. These plans are reviewed regularly and updated as needed to reflect progress, changes in behavior, and evolving goals. Our staff are trained in Positive Support Strategies, person-centered practices, and prohibited intervention avoidance—ensuring we meet both the letter and spirit of Minnesota's DHS requirements.
We use data-driven behavior tracking, daily documentation, and incident reporting tools to monitor behaviors, triggers, responses, and outcomes. Staff receive ongoing training in de-escalation, communication alternatives, and least restrictive intervention techniques, as mandated under 245D. Crisis services and mental health coordination are also integrated when clinically indicated, and we maintain strong partnerships with county case managers, mental health professionals, and family members to ensure wraparound support.
Our goal is to help each person reach their highest level of independence by building coping skills, emotional regulation strategies, and trust in those who support them. We promote dignity, autonomy, and community inclusion while strictly avoiding punitive or restrictive methods.
