At Belong Health Care, we offer Individual Community Living Supports (ICLS) to help adults live independently in the community while receiving flexible, individualized assistance. ICLS is a home and community-based service under Minnesota’s Elderly Waiver (EW) and Alternative Care (AC) programs.

At Belong Health Care, our Individual Community Living Supports (ICLS) program is designed to help older adults maintain independence while safely living in their own homes or with family. This service is available through Minnesota’s Elderly Waiver (EW) and Alternative Care (AC) programs and provides flexible, person-centered assistance in key areas of daily living. ICLS is ideal for individuals who do not yet need full residential care but benefit from structured, supportive services to manage their health and home.

Our ICLS services cover six core areas, including health and safety monitoring, household management, adaptive skills training, assistive technology support, behavioral support, and community engagement. We work closely with each individual to build daily routines, encourage independence, and reduce the risk of isolation or decline. Whether it’s organizing medications, maintaining a clean home, or practicing new life skills, our goal is to empower clients to live confidently and with purpose.

Each support plan is tailored to the person’s needs, goals, and preferences, and delivered by trained staff who build trust through consistency and compassion. We also coordinate closely with case managers, families, and healthcare providers to ensure continuity of care. With ICLS, clients receive just the right amount of support—enough to thrive while staying in the community they call home.
