Long-Term Disabilities Care
Our care team works collaboratively with residents, guardians, case managers, and healthcare professionals to develop individualized service plans that address each person’s goals, preferences, and support needs. Whether assisting with mobility, communication, personal care, or behavioral support, our staff deliver services with dignity, patience, and respect. We also assist with adaptive skill development and daily living activities, helping residents grow in confidence and capability.
We support a wide range of disabilities, including traumatic brain injury (TBI), cerebral palsy, autism spectrum disorder, Down syndrome, multiple sclerosis (MS), and intellectual disabilities. Our homes are accessible and designed to minimize barriers, with modifications and assistive technologies available to promote safety and independence. We also coordinate therapy services, transportation, and medical oversight to ensure holistic care.
Beyond daily routines, we focus on meaningful social and community engagement. Residents are encouraged to participate in group activities, volunteer programs, spiritual life, and recreational outings tailored to their interests. Our goal is to ensure each person feels valued, included, and empowered to lead a fulfilling life—regardless of the level of support they require. At Belong Health Care, long-term care means building lasting relationships and supporting every individual’s right to live with purpose and dignity.

We are here to help when you need us
We believe in empowerment through support. Our goal is not only to provide care—but to help residents live fulfilling lives, make choices, participate in the community, and maintain strong relationships with loved ones.
